Registration & Medical History
Your contact details together with your medical history, medications and allergies. The most important of the three — bring it completed.
Treatment Consent Form
Read and sign before treatment. Bring any questions about it to the consultation — it is meant to be discussed, not just signed.
HIPAA Consent Form
Your privacy rights and how your health information is handled and shared.
No printer? That is fine — arrive about ten minutes early and complete them at the practice.
On the day
What to bring with you
- Your completed forms.
- Photo ID and your insurance card.
- A written list of all medications and supplements.
- Any recent X-rays, or your referring dentist’s contact details.
- A driver, if you are having sedation.
